• Range of Motion (Spinal Alignment and Range of Motion Measure | SAROMM training and testing presentation with audio and video | Scoresheet for SAROMM Video Test)
• Endurance (6-Minute Walk Test)  (Early Activity Scale for Endurance, link to: Portuguese translation, link to Turkish translation, link to Spanish translation)
• Overall Health (Child Health Conditions Questionnaire, parent completed) (link to Spanish translation)
• Participation in Family and Recreational Activities (Part One - Child Engagement in Daily Life Measure (Version 2) parent completed) (link to Spanish translation)
• Performance in Self-Care (Part Two - Child Engagement in Daily Life Measure (Version 2), parent completed) (link to: Arabic translation and link to Spanish translation)
• Services (Services Questionnaire, parent completed)
• Gross Motor Function Classification System - Expanded & Revised (GMFCS-E&R), parent & assessor completed
• Manual Ability Classification System (MACS), parent & assessor completed
• Communication Function Classification System (CFCS), parent & assessor completed
